A US form is not a Canadian form.

Canadian courts reject documents that cite the wrong jurisdiction. An Alberta court will not probate a will built around California's Probate Code. The Ontario Landlord and Tenant Board will not enforce a notice that cites the Texas Property Code. Province-specific compliance is not a nice-to-have.
